Dan Goldberg
President and Managing Director

Dan Goldberg is President & Founder of Vital Care Partners.  Mr. Goldberg is a Certified Senior Advisor with 30 years of experience as a problem-solving finance professional with a background in investing, analyzing and providing advice to institutions and individuals.  His passion for healthcare stems from his personal journeys through the healthcare maze helping his own family members.  And his desire to help others originated from his mother, who is a retired social worker, as well as the experience his son had shadowing an autistic child at camp five years ago.  Dan believes that through a team approach they are able to achieve the best outcome and quality of life for their clients.

Mr. Goldberg graduated from the Wharton School of the University of Pennsylvania and the Graduate Business School of New York University.  He holds various professional designations and certifications and serves on the Board of the Jewish Opportunities Institute in Israel, which specializes in providing social services to immigrants.  He also has been a member and volunteer with NAMI (National Alliance for Mental Illness).

Susan E. Goldberg
Registered Nurse Advocate

As the team leader Registered Nurse, Susan Goldberg, RN, Ed.M., MA, COHN-S brings more than 30 years of experience as a clinical nurse in both acute and chronic settings, nursing educator and clinical manager. Susan received her Bachelor’s of Science degree from Syracuse University and has a dual Master’s Degree in Nursing Administration and Nursing Education from Columbia University. Susan also earned her Certification in Occupational Health Nursing and is a certified PRI and Screen Accessor for the state of NY. Susan has also earned a Master’s Degree in Criminal Justice and Counter-terrorism Studies from John Jay College of Criminal Justice in NY.

Susan’s interest and involvement with health care advocacy grew as direct result of having personally been the health care advocate and proxy for her aging parents and an uncle who suffers from Dementia. Her in-depth knowledge and understanding of the health care system enabled her to navigate through and access all the benefits and services available. Having an advocate who was well-informed enabled her family members to benefit by providing them with a better quality of life while capitalizing on existing funding. Susan volunteers her time with the USO of New York, NY Medical Reserve Corp and has volunteered for NYC Marathon Medical Corp.

Sandra Garrett
Registered Nurse Advocate

Sandra Garrett RN, CHPN is a Nurse Advocate with over thirty years’ experience as an RN and possesses strong clinical and organizational skills. She received her Nursing degree from Saddleback College in Southern California and is a certified hospice and palliative nurse specialist. Sandra has a diverse background in intensive care nursing, high-tech Infusion therapy, home health care, and hospice and palliative nursing. Across the health and wellness continuum, she collaborates with physicians, case managers, social workers, patients, and families to improve patient outcomes and well-being. Sandra has also provided many families and patients with compassion and care at the end of life. Beyond her diverse clinical experience, she has participated as a clinical nurse in international medical missions to East Africa and the Philippines providing nursing care and medications to the impoverished.

Mary Sears
Registered Nurse Advocate

Mary Elizabeth Sears is a dedicated healthcare professional with over 20 years experience providing behavioral health care in the capacity of Personal Care Aide, Certified Nursing Assistant, and then advanced her career to become a Registered Nurse. She worked as a staff nurse in inpatient, outpatient, and Comprehensive Psychiatric Evaluation Programs where she was responsible for overall patient care and assessment. Mary is profoundly grateful for the opportunity to support the wellbeing of the very vulnerable population with mental health issues.  In addition to her professional career, Mary loves to help people and enjoys giving back to her community both in the United States and internationally. She has organized and facilitated many health fairs for communities in the US and has volunteered her nursing expertise on overseas mission trips.  Mary is a member of the Life Enrichment Adventist Ministry where she serves as an Elder and Children’s Ministry Coordinator. She is also a valuable Board Member for a 501c3 organization that focuses on education and spiritual development of youths in Guyana.

Yvonne Secreto
Registered Nurse Advocate

Yvonne Secreto is an RN Case Manager who brings more than thirty years’ nursing experience as a clinical nurse to a diverse patient population with expertise in areas such as oncology, pediatrics, trauma care, hospice care and psychiatry. During that time, she has worked for varied institutions in the medical field including hospitals, insurance companies, Certified Home Health Agencies and is also a lecturer and facilitator at City College of NY. Yvonne earned her BA in Healthcare Administration from the City College of New York (CUNY) and her Associate in Science RN from Catholic Medical Center School of Nursing. She is passionate about health and wellness and teaching advocacy to patients and their families. Yvonne is also an author and is bilingual (Spanish).

Jessica Wolfarth
Care Management

Jessica Wolfarth, LMSW, LPN, ACHP-SW, CDP is a Medical Social Worker and a Certified Dementia Practitioner who has provided prevention and clinical intervention services to patients and their families for over a decade. Her skills include facilitating intakes, ongoing assessments, individual, group and family counseling, crisis intervention, education workshops, discharge planning, referrals, utilization of community resources, long term planning and housing and helping with insurance applications and financial services.

Ms. Wolfarth received her Master’s in social Worker from Fordham University in May of 2007 and has a dual concentration in clinical and administrative management. Her experience with hospice and home care lead her into seeking a nursing degree to provide more assistance to her clients and families. She obtained her Licensed Practical Nurse Degree in August, 2017.

RoseAnn Simeone
Client Care Manager

RoseAnn Simeone is the Client Coordinator for Vital Care Partners. RoseAnn has over 35 years experience in the healthcare, business and customer relations field. RoseAnn is Certified as a Medical Assistant, End of Life Doula, EFT Practitioner as well as being an Advanced Reiki Master and licensed Health and Life Insurance Agent. RoseAnn has been a longtime caretaker to the sick and elderly, advocating for their specialized care and medical needs.She has worked with the aged in Assisted Living and Nursing Homes as a Programming Assistant where she organized and implemented programs designed to meet the individual needs of the residents. RoseAnn has also created workshops such as “The Journey Home”in senior centers and“Tapping for Kids, Teachers and Parents”in elementary schools in St. Thomas, USVI. She has also done business workshops designed to reduce stress and anxiety,as well as speaking at various colleges on the many faces of grief and the benefits of having an End of Life Doula. She is currently implementing and assisting in “Death Cafes”through a local church to help open the conversations on death and dying.

RoseAnn has donated countless hours volunteering her time at Behavioral Health Group, Parker Jewish Hospital in their Palliative Care and Hospice Departments, the Coalition Against Child Abuse and Neglect,American Cancer Society, PanCan creating the 2017 Purple Light Ceremony in Nassau County, the Lustgarten Foundation, and continues to serve on the Executive Board for the Nassau County Law Enforcement Exploring program. RoseAnn has received a Citation from County Executive Thomas Suozzi for her work with the Lustgarten Foundation. She has spoken at the Dr. Martin Luther King, Jr. 25th Birthday Celebration Event, the Center for Homeland Defense and Security Consortiumin Monterey, CA,and has received two Citations for her devotion to S.T.R.O.N.G. Youth.

 

Aimee B. Stoopler, BCPA Client Care Manager



Aimee is a graduate of the University of Maryland with a B.A. in Health and Sciences. She is a proud member of the National Association of Healthcare Advocacy (NAHAC) and the Alliance of Professional Health Advocates (APHA). In 2018, Aimee completed the Healthcare Advocacy Certificate Program from the University of Miami and is nationally Board Certified. Her mission is to help people age and transition with dignity and grace. Aimee has worked and volunteered in the Senior care community for over twenty years. Aimee’s passion for healthcare advocacy was further realized after her daughter was hospitalized for a major, life-threatening complication arising from an otherwise routine surgery. This ordeal empowered her to advocate for those going through their own health journey.
